What determines the function of each key or button in FSX? Is that all done in Standard.xml? Can it be edited manualy?

Yes, all keyboard key and joystick/controller assignments, including default and the edits that you make in Settings>Controls, are stored in the standard.xml file (unlike FS2004, where they were stored in the fs9.cfg file).

I suppose you could edit it manually if you know all the required string values. I would suggest using Settings>Controls to avoid errors.

At that time [1909] the chief engineer was almost always the chief test pilot as well. That had the fortunate result of eliminating poor engineering early in aviation.          Igor Sikorsky

The key commands and there corresponding actions can be found by opening up FSX, clicking on the Settings button, clicking on Controls in the Other settings box, then click on Buttons/Keys.
