I've been in cases where I am about to die so I need to eject, but when I do the parashute won't open. It says press O to open parashute but it never opens. Is there a altitude limit to open it or what?

I think the game is just simulating the "parachutes don't always open" idea. As I recall it also simulates your screaming.

But after I hit the water I get the message saying I died, and I never hear the screaming at all.

I hit the "O" key repeatedly until my chute either opens or I get a message saying it didn't open.  For me hitting the key only once didn't seem to trigger it.  About 10% of the time your chute won't open but you'll see a message saying so.

Also make sure your planes is slowing down just before you parachute. If you AC is to fast for you to parachute your chute like in real life will not work properly!

Well I found out the problem. I changed my eject button to a remote, so after I hit the eject button I pressed "O". But now I have another question. Do you always get captured by the enemy when you eject in the middle of a mission?

No, it depends on where you are.  If you are behind enemy lines, in other words you have crossed a Front Lines designator and are on the enemy side, then you will be captured.  If you bail out over friendly territory you usually get piked up by an allied sub.
