True Andrew, but sometimes stuff breaks at the most inopportune moments no matter how much preparation, inspections or checks are carried out. Sh#t happens would be rather a hard way of putting it, but unfortunately sometimes it does.

Sounds chillingly like the Kevin Costner movie "The Guardian". Very unpleasant. It isn't the most pleasant of things normally anyway, with just a strop under your shoulders, and no physical attachment.

Something similar has happend in my country not long ago.
A man was hit by a mine (he entered an area that has warning signs about it) and got injured, and after the rescue helicopters got him up he fell (from about 20/30 meters).
