Arrgh, that one is terrible; I remember it.
Here's what I think:
1)High density altitude
2) Qverloaded
3)Stupid people too close too the "runway"
4) Pilot did not wait quite long enough before pulling up... he was doing okay at one point, once he got the yawing under control, then he pulled back and it stalled in the classic fashion.
But behind all of the above, I think, was very bad "get-there-itis"... airborne medical evacuations, even with helicopters, often go bad because of the sense of urgency. People lose their focus in all the excitement.
The lady in front may have been a factor, but it was not a very good plan to begin with. It would be hard to tell her to stay behind for safety's sake, but...between her weight and her being so upset, it might have made a big difference.

I ran few test (This is not a sick joke) waching the video and using the 172 of FSX ,and the only thing that match in the same exacly way ,I mean the turn to one side of the plane after lift the wheels of the ground, the way its stall , it does exacly the same way that plane falls . if we overweigh the plane ,try the same with the weight of 4 passengers 120-140lb each, then ADD the Cardio machine and you seen the size of the oxigen tank?, like 300 lb the cardiomonitor then add the bagdage some more ,then you have it, they were so worried about the sick of the child ,the pilot didn't realize it..
I never seen this video before and it shock me ,for me was overweight as you said ,the runway (if that can deserve the name) and maybe the exitment wich blind the judge of this pilot,
Pilot error..

And the density altitude. You know that area better than I do, maybe- this was in the hills, right? On a hot day?
Anyway... it was very disorganized, and now they're all gone, even the kid. Very sad.
